By ordering this service and agreeing to prepare the sample, the user agrees to waive and release Labosport, SYPAC and their employees from any claim, demand and cause of action for any injuries or death to any person, losses or damages to any property (including without limitation, equitable relief) that he may now or hereafter have a right to assert against such parties as a result of use of, or reliance on, this testing program. Labosport and SYPAC do not make any warranty (expressed or implied) or assume any liability or responsibility to the user or other third party, for the accuracy, completeness or use of, or reliance on, any information contained within the report, or for any injuries, losses or damages (including without limitation, equitable relief) arising from such use or reliance.
